This electronic form serves as a legal and binding contract between the Adopter (Hereinafter referred to as “you” or “the adopter”) and All Fur One Pet Rescue & Adoptions (hereinafter referred to as “AFO”). We do not allow modifications or amendments to this contract.

Adopters need to consider their individual situations and resources when deciding to adopt a pet, using their best judgment to make an educated and appropriate decision regarding adoption based on the household and lifestyle in combination with the information disclosed, exploring, and heavily weighing the possible benefits and limitations regarding the pet in consideration of the following terms as discussed within this contract.

Submission of this form binds this adoption contract in full as of the date of the adoption AND all adoption fees have been satisfied AND physical transfer of the rescued pet into your possession. Until such time, the contract is null and void.

Terms & Conditions*
- IMMEDIATELY UPON ADOPTION: ADOPTER RESPONSIBILITY AS LEGAL OWNER By adopting and taking possession of the pet, the adopter is responsible for providing for the health and welfare of the pet. This includes, but is not limited to, adequate food, water, exercise, shelter, long term preventative veterinary and medical care, training and love. The adopter shall provide and be responsible for all routine and acute veterinary care for the life of the pet. This includes annual well visits, vaccinations, care for illness, injury and disease.
- IMMEDIATELY UPON ADOPTION: ROUTINES REDUCE RETURNS You agree to keep your new pet on a strict schedule as recommended by AFO to ease the transition into a new home, benefitting both you, your family and your new pet for a minimum of two weeks. This should include (but not be limited to): crating your pet when unsupervised to prevent destructive behaviors, providing opportunities to use the bathroom outside no less than every 3-4 hours, feeding meals at the same time each day, keeping pets separated when unsupervised, limiting escalated play between household members and household pets, requiring "quiet/alone" time no less than twice a day for 1-2 hours per session, minimizing introductions to new experiences such as dog parks, pet stores, family/friends and/or other animals.
- WITHIN 24 HOURS OF ADOPTION: ACTIVATION OF TRUPANION PET INSURANCE You agree to immediately (within 24 hours) sign up for the one-month free pet insurance. If you fail to sign up for this free month of insurance, you willingly accept the risk of incurring unexpected veterinary/medical costs within the first month of adoption for which AFO is not liable or to be contacted for reimbursement of such incurred costs.
- PROMISE TO ALTER APPOINTMENT (IF APPLICABLE) AFO pays for the spay and neuter of all pets adopted from our organization. If the pet has not yet been spayed or neutered at the time of adoption due to a medical condition or unfit to alter on the schedule alter date as advised by AFO’s Partnering Veterinary Team, you agree to adopt the pet in its current unaltered condition with the promise to alter at the first available appointment at the New Jersey Spay and Neuter Clinic in Holmdel, NJ. AFO will provide you with the appointment date at adoption. You are required to keep this appointment and no changes will be made without approval by AFO. "Breach of Adoption Contract" will result in repossession of the rescued pet WITHOUT A REFUND OF ADOPTION FEES if confirmation of alter date is not provided to AFO within said timeframe.
- WITHIN 14 DAYS OF ADOPTION: INITIAL VET EXAMINATION REQUIREMENT Within fourteen (14) calendar days of adoption, you are required to have the pet examined by a licensed veterinarian. For puppies, it is critical to have this completed as they will likely have vaccinations due within this time period and not completing as required can potentially mean starting the entire vaccination series over leading to unnecessary and irresponsible over-vaccination.
- EXPIRES AFTER 14 DAYS FROM ADOPTION: REFUNDS DUE TO UNEXPECTED MEDICAL CONDITIONS Should the veterinarian determine the pet has any medical condition, with the exclusion of testing positive for intestinal parasites, you may choose to return the pet to AFO for a full refund of the adoption fee only (not incurred veterinary costs for veterinary/medical care), provided the veterinarian includes a written diagnosis to AFO and the date of examination was within the 14 calendar day window. If you choose to keep the pet, all expenses incurred on or after the date of adoption shall be your sole responsibility as the owner.
- REQUIRED FOR 30 DAYS AFTER ADOPTION: OBSERVATION OF PETS & CHILDREN You agree to observe your adopted pet for a minimum of one month, in a variety of situations/environments, before establishing a full determination of its personality and behaviors and during this time period will limit introductions to children under 15 and further agree your adopted pet will never be left unattended/unsupervised with any child under the age of 10 years old.
- 30 DAYS AFTER ADOPTION: FECAL TESTING It is common for pets to have intestinal parasites and fecal testing/deworming is considered routine preventative care. AFO requires that a fecal test be performed by the licensed veterinarian (home tests will not be accepted) after 30 days from last deworming. AFO will provide one round of deworming products for you free of charge for worms, giardia and/or coccidia if the animal tests positive. If you do not get the fecal test within this period, you will be responsible for all deworming treatments. AFO will not reimburse the adopter for products purchased by the adopter at your veterinarian’s office or elsewhere.
- STATEMENT OF VETERINARY CARE AFO provides age based preventive veterinary care as directed and deemed appropriate by our Supervising Veterinarian of Record or other Veterinarians that have cared for the pet while in our possession. All veterinary records and medical care/treatments of the pet you are adopting only reflects the time period the pet was in AFO’s care and under AFO’s ownership and will be released to you upon adoption. AFO willfully surrenders all such records and grants full permission to contact all listed veterinarians and consents to the release of any and all records by such veterinarian providers pertaining to this pet.
- STATEMENT OF PET QUALITY AFO has rescued this pet and does not have reliable information as to age, breed, health, training, registration, or anything else important to the history of the pet. While AFO will disclose any information offered to us during intake of the pet, AFO’s statements regarding the pet being adopted solely represents information gathered while in our care and does not represent any past behaviors or veterinary/medical care nor does it represent any indication of future behaviors or veterinary/medical care required beyond preventative routine care.
- STATEMENT OF PET BEHAVIORS While observed behavior at AFO as a whole is a reflection of personality, a pet’s behavior at any given time is heavily influenced by it’s emotional state, it’s stress level and the specific environment. The behavioral profile of the pet you are adopting was determined through observation only while in our care and does not represent any past behavior nor does it represent any future prediction of future behavior.
- STATEMENT OF BREED MIX AND GENETIC MARKERS AFO has zero ability to confirm or otherwise guarantee breed/breed mix for any pet in our care as we do NOT DNA test. At best, AFO will inform potential adopters of the breed listed on veterinary paperwork – to which is it to be understood this breed listing is a “visual guess” and is to be considered ZERO percent reliable or accurate. Further, all adopters should be prepared to willingly accept their dog and continue to love their dog regardless of future DNA testing results as compared to breed type listed on veterinary records.
- EUTHANASIA POLICY The pet may not be euthanized unless it is accompanied by pain and suffering with the concurring opinion of a licensed veterinarian. AFO must be contacted in this event. Also, AFO must be informed of any potential future euthanasia associated with genetic conditions or degenerative diseases which may impact any other litter mates of the pet.
- SURRENDER POLICY If for any reason you cannot keep the pet you MUST notify AFO immediately. AFO shall have 30 days to relocate the pet and during that 30 day period, if the pet cannot remain safely in your home, you agree to pay any boarding charges that may be required to safely house the pet until we have an open kennel or available foster. At any point in time the adopted pet is returned to AFO it is considered a surrender. At the time of surrender, you forfeit your right to request a refund of any adoption fees, regardless of the time between adoption date and surrender date.
- RIGHT TO REPOSSESSION The pet shall not be tied or tethered outside, left in a run or a pen, left outside unattended, used for research purposes, gifted to another person, sold, traded, or surrendered to any agency, shelter, person, school or organization. If the pet is a cat, it shall not be let outside unattended. Adopter also agrees not to mutilate feline pets by declawing or mutilate canine pets by having its ears/tail cropped. AFO retains the right throughout the life of the pet, to reacquire the pet at any time if the terms of this agreement are suspected to have been breached or if any misrepresentations have been made by you.
- VETERINARY RECORD CONSENT AFO will require you to provide the contact information for the veterinary practice you will be taking your pet to. This signed contract will serve as your authorization giving permission for AFO to contact your listed vet and discuss all medical/veterinary history and/or request copies of your records if you have not altered your pet and were required to do so OR if AFO is notified or suspects you have not satisfied the required initial veterinary exam OR if AFO is notified or suspects neglect or abuse, OR if you surrender your pet to AFO.
- FINAL ADOPTER ACKNOWLEDGEMENT You shall indemnify and hold harmless AFO and its representatives from any and all claims of liability for the conduct of the pet on and after the date of this agreement. All pets adopted are the sole responsibility of the adopter whose signature appears below. You agree you are adopting the pet described within this contract based on your observed personality of the pet, your individual situation, and resources available plus your best judgment in making an educated decision to adopt this pet as appropriate for your household and furthermore you acknowledge you are not adopting this pet based purely on aesthetics, breed or an otherwise bias assumption of the dog’s appropriateness for your household. In addition, you agree to do your due diligence in providing the necessary training and taking the necessary, responsible, precautionary measures to ensure the safety and well-being of the pet and all others (animal and human) that come into contact with the pet.
